You can try Unite PBX http://www.unitepbx.com its a software based VoIP solution and its based on top of Skype's existing reliable services and skype has many such Subscription schemes as well.

I am looking for a VoIP provider that will let me set my own prices, but bill my customers for me based on their usage. (Similar to how Mosso - the on-demand arm of Rackspace - does for web developers that use it's service to host their clients' websites)
