Accessing Corporate IP Phone from Home

My company has just moved to an IP phone system for the office but I actually work interstate from home.

Is there a way to connect add an IP phone to my home network and use it to connect to the corporate VOIP server so that I can
have an extension just like the staff who are actually situated in the office ?
