IAX softphone for Windows, Linux & Mac OS X

- The Mac OS X version (I don’t know about the other editions) is buggy and quirky. It crashes, and it messes up enough that you often need to log out of Mac OS X and log in again to be able to use IAXComm again. But IAXComm is the only IAX client available for Mac OS X. So live with it.
Asterisk Setup Notes
- All IAX accounts that are being accessed by IAXComm must have `qualify=no` set, or you will receive Peer is now UNREACHABLE messages in Asterisk and the client will no longer be able to receive inbound calls.
- Accounts must not have the username parameter set or authentication will fail.
