I have a phone system that has had to have the fwconsole restarted twice in the last two days. After looking through the logs, this occurs right before I start having problems:
[2020-12-15 10:03:40] WARNING[5416][C-000069e4] channel.c: Exceptionally long voice queue length queuing to Local

Other things that I notice is that SIP doesn't reply and all phones drop their registration. Our SIP provider receives a 503 and 486 error. Reloading chan sip doesn't appear to solve the problem.

Current Asterisk Version: 13.28.1

Thanks,

I think I'm correct in saying Asterisk13 is no longer supported here, tho if someone has a quick fix they may pass here soon.
More Info needed:
You don't mention which platform or is it pure Asterisk?, in the cloud /bare metal nor which O.S. - there may not be many Astrisk 13 s running hereabouts, and I don't know the answer but with more info it may help someone to get to you.
Be aware you might find something on the Incredible Wiki -

I am running Incredible PBX 13.0.197.22 on a CentOS. It is a physical HP server with a direct Ethernet handoff via SIP from the dial tone provider.

Thanks for the assistance.

Have you done a module update in recent history? ADMIN > Module Admin > Check Online (may be slightly different on FreePBX 13 but you should be able to figure it out. FreePBX 13 is deprecated so I'm not sure module updates are even available anymore.

If your modules are current and this is happening, then there may be something weird coming at you from your dial tone provider. Also, if you are using PJSIP, that was extremely unstable in some older Asterisk/FreePBX versions. The bugs are worked out in current versions.

All-in-all, the best advice is to upgrade your IncrediblePBX installation to current versions. Sadly, there is no direct upgrade path so you need to install on another server or VM instance and cut and paste from old to new.

ADMIN > Module Admin
From memory I reckon those menu items are the same as in Asterisk16
so here is 16:-

If there is a checkbox "show only upgradeable" check that 1st then click process to see the upgradeable ones. Easiest to click 'Upgrade all' and accept the dialog with Install
(I think).

Just a lonshot possibility you may have a working backup where queue was OK ?

I remember an update script sort of equivalent to
root@incrediblepbx:~# update-asterisk16

Called probably something like '/root/upgrade-asterisk13-to-latest' but before running an Asterisk upgrade you should do
Code:
/root/incrediblebackup13 #if that is what it is called in13
#then
/root/upgrade-asterisk13-to-latest
#or whatever that is called

I agree with @kenn10 your best bet may be to start again with a new version.
some of the settings can be 'copied and pasted' across, so that is still a painfull process.

As the are saying this side 'have a small Christmas' due to Covid-19 fears.

I've seen this once beore when the system gets really busy. If you run top during a problem period what is burning the CPU? Is there anything else in the logs? When you say it crashes is there a seg fault or something in the logs? how big are the log files?
