I have cloud servers in a few places but keep running into the same problem
and I feel like it's somewhere within my pfsense setup. My local VMWare setup
works fine but there is a big advantage to the cloud as I see it.

I have Skyetel pointed properly to the ip address in the cloud

I have my port forwarding (UDP) 5060-5082 pointed to my cloud ip
as well as 10000-2000.

I have outbound NAT mode directed to my ip/32

My lines won't stay registered - am I missing a setting? Suggestions
appreciated.

Welcome to the forum.
I am not sure what you mean by "outbound NAT mode directed to my ip/32". Default outbound NAT settings of pfSense should work just fine for your PBX.
Your WAN to LAN port forwards look fine (except for the typo on 20000).
The only thing that I can think of is that your cloud service (the VPS you are using) has a network interface which is dropping the network connection periodically.
My company has several locations in the US, in EU and elsewhere. We also have many home based workers. We use pfsense across the board with little difficulty. But we also host our own VPSes on our own co-located hardware for better security and reliability.

Edit:
After reading @Eliad 's post below I just wanted to clarify that you're trying to protect your VPS based PBX with a VPS based pfSense firewall, right? (If so, you would have one VPS installed with pfSense with dual network interfaces, and another VPS at the same facility with local net connection to the pfSense VPS).

I understand you have PBX in the cloud and the phones are local. The local network has pfSense for firewall. In this case you don't need any of the port forward in pfSense.
From my own experimentation if you enable port forward the phones will have trouble registering. You need those port forward if you have a local PBX.
I tried a local PBX and also a cloud PBX through a pfSense and I had trouble registering the phones when connecting to the cloud.
Perhaps somebody here knows more and will guide you on how to set up pfSense to make it work for both.

Need more information regarding the setup and what you're wanting to.

Is your PBX in the cloud and just your phones behind PfSense?

If so you don't need to set any port forwards or rules.

I've used PfSense this way for years with aastra, yealink, polycom and now obi phones.

I only configure port forwards and rules when the pbx is self hosted behind the Pfsense FW.

Edit: guess I should have read Eliad's post, he covered this.
