NEED MORE INFO Wierd Call Forwarding Issue

We are running into a weird call forwarding issue and I am not sure where to start to resolve this.

So one phone keeps turning on its call forwarding feature by itself. Once this phone turns this feature on any incoming calls that have this phone in its ring group the call will automatically go to voice mail. It rings the ring group once but does not allow us to answer. .

Do you think this is a problem with the phone or the pbx setup/system? I am running Cisco IP 303 phones.

Please post a call log of a relevant affected call.

Not quite. That looks like the CDR trace from FreePBX. We'd need the full call log.
Open SSH to the box, run 'asterisk -vvvr', place a call, and paste the (sanitized) output here
