Aastra i57 won't ring from IVR or Queue

I recently setup two new Aastra i57 phones in our office. Did the setup exactly as provided from nerdvittles site, "Thanks again guys". All features seem to be working fine, we can call out, and can receive calls if extension is dialed from another phone in the office. The problem is if someone dials the Aastra extension from our IVR they just get a busy signal (no ring). Also, if the phone is logged into a queue and a call comes in it also will not ring. I have verified the phone is in fact logged in and if you look at the other phones in the office the light for this extension does flash as though it is receiving the call, but no ring. Has anyone ever seen this and could maybe help out? Thanks. :banghead:
 
Here is what I'm getting in the log when trying to dial extension from IVR. :banghead:
