Audio Quality lost on Xfer --- Cisco SIP

Customer is telling me that if they transfer a call most (Not All) transfers complete but the audio path is lost and they end up hanging up and calling the party back. If they park and then pickup the call all is fine.

Has any seen anything like this. Thinking an update to a latter flash "POS3-08-9-00" might make a difference ??

But any input would be appreciated..
