Calls in queue disconnect when answered

We have two phones that we want to both ring on inbound calls, so I have an inbound route setup to point to a queue that has the extensions of both phones.

On occasion (maybe once or twice a day) we get a phone call that when answered immediately hangs up on the first phone but continues to ring on the second phone. In these cases we must answer on the second phone and transfer the call back to the first.

Any ideas?

Maybe this helps, in the Call Report it says the first phone is BUSY.

I'll try using a Ring Group and use the latest Firmware..
