Can't find Custom Apps.

I'm trying to add Caller ID to my cellphone when it is forwarded from PIAF as in Introducing the Stealth AutoAttendant for Asterisk 1.4 and FreePBX.

I can add the requisite lines to the extensions_custom.conf file, but I don't see where/how to change the destination to use this custom app that I have created.

I have the custom apps module loaded.

Can someone point me in the right direction?

Add it as a "custom destination" under tools - then it will appear in the IVR choices.
