CID Superfecta not working

This is not in the same order as the debug information you posted above. In order to help I need a complete and consistent picture of what is going on. I also need you to disable the sources one by one as I instructed so we can narrow down the issue to the root cause.
 
Found

I saw that in the sources there was listed Astridex then 2 lines down was Asteridex. The first is what gave the notice of
result
scrollup.gif
took 0.0003 seconds. with no source listed. I removed and will test now....

It is showing that it is setting the name as the phone number.
