CID Superfecta Problem

I've got a new install with the DOCTYPE error in the CID from CIP Superfecta. Here's the log:

I've put the MAINT : PASSWORD in the general settings page but it isn't helping. In the debug on the setup page everything works fine. It just doesn't translate when the call actually comes in. What do I need to check permissions on?
 
The only place I see it is in the General Options section of the CID Superfecta page. Is that where you're referring to? Because I've done that.
 
I checked that one and it's set up with the right credentials as well.
 
Yes I can ping localhost. Yes, everything is set up properly. ;(
 
I don't think I even have GV installed since I can't find it. Where would it be in FreePBX if it did exist?

In CallerID Lookup Sources check that the source type is set to "internal" and not "HTTP".

I think that is the right way, I do not have access to a working 2.2.5 system to compare with at present.
 
Doh, It would be worth checking the apache error logs as well.

I don't know why but it seems to me it's some form of permissions issue. Since I can navigate to the page and get the right info and since I can debug and get the right info, it seems like maybe Asterisk doesn't have the right permissions to call it while Apache does. All the files in /var/www/html/admin/modules/cidlookup/ has asterisk:asterisk for permissions.
