I have a PIAF incredible PBX 11 EC2 instance and remote phones behind pfsense routers. I am able to place callers on hold, but when I go to retrieve the call I can hear the callers voice, but they can not hear mine. I use cisco 525G2, 508G, 942, and 303 phones and the problem arises on all the phones.

I should also say that this does not happen every time. I noticed that on my PIAF sever, my phones would become unreachable when idle for several minutes (until they reregister). This prompted me to turn registrations to every 20 seconds (registertimeout) which corrected the issue.

I have since set rtpkeepalive to 20seconds, but this does not seem to be working.

My attempted fixes include:
1) change router to keep routers states as long as possible (pfsense terms "conservative")
2) static outbound nat for port 10000-20000 (my RTP ports)
3) Keepalive on
4) Rtpkeepalive to 20 seconds (keepalive enabled on phones)

I do not know where to go from here. There is an additional setting on the cisco phones for nat mapping enable (currently set to NO) that I could change, but I do not completely understand its function. Can anyone offer some advice????

My guess, without seeing anything, is that the re invite has the wrong IP address.

Thank you both. ReInvite is set to NO on the server. I am guessing it should be set to YES?

Should I also set all endpoints endpoint to canreinvite=yes, currently they are set to canreinvite=no?

My expertise is mainly with 7900's so not too familiar with idiosyncrasies of these handsets, however if this was a problem on 7900's I would say make sure all transport is TCP and not UDP. That causes similar problems to what you describe.
