We've just discovered that the iptables setup for Fail2Ban in PIAF 1.7.5.6 is missing a component that actually manages Fail2Ban and triggers the lockout mechanism. We'll have a fix included in update-fixes shortly and will post the notice here.
NOTE: This only affects PIAF 1.7.5.6 installs, not 1.7.5.5! If you're running Incredible PBX with PIAF 1.7.5.6, it's NOT A PROBLEM. Do NOT overwrite the Incredible setup which is much more complex to support White Lists!
If you're in a hurry and want to do it yourself, here's how after logging into your server as root:
