QUESTION Hosted v On-Premise: Why does hosted work better in an un-managed environment?

Is it my imagination or do Hosted Providers get better results in situations where there is no QOS or any other traffic shaping?
I have a some customers on Comcast Cable Broadband service; two of them have PIAF boxes with VOIPMS trunks and nothing to prioritize voice on their LAN. From time to time they will have issues and I will have to install something to mitigate theses problems.
I have three other customer for whom I provide non-voice services and who happen to have hosted Phone service with other providers. These three customers never seem to have issues despite also not having anything but the Comcast Router in-place.
It seems counter intuitive!
What are the hosted providers doing that I'm not???
 
When companies like Comcast or Cablevision provide phone service it is my understanding that their VOIP traffic is prioritized on their network through various VLAN configurations.
 
Do you use G.729? They probably do, and that codec will give better results over a congested, unmanaged Internet connection.
