I am using a PBXIAF1.8 purple server at rentpbx temporarily. Most everything works fine except for a couple of weird issues:

1. All extensions are remote and are located at 3 separate locations.

2. When calls to one remote extension are picked up the phone keeps ringing on the other end if it is an outside call. If it is an inside call, the phone stops ringing but there is no audio received at the remote end but the caller can hear the remote audio. I.E. External call to ring group rings ext 514. 514 is picked up but external calls keeps ringing and never responds to the pickup. Other extensions can pick up the call just fine. Extension 514 can call internally and externally without any problems with audio or signalling. Second Example: Extension 500 in office A calls extension 514 in office B. 514 gets no audio but 500 can hear 514. 514 can call 500 with no problem on either end.

3. Transferring calls from one extension to another fails about half the time. This happens with calls in the same office.

I would appreciate any troubleshooting ideas on where to start. I have checked the ports and externalip settings and such and really dont know what else to do.

What brand of phones are you using? Take a good look at the phones config and make sure it matches the rest of the phones. How many phones are at the location with ext. 514 and what kind of router is there?

John - the phone that is at extension 514 is a SNOM 300. I have a Linksys SPA 941 on my wife's desk a few feet away that works fine. The router is a DLINK DIR 601. The phones worked fine under Asterisk 1.4 on my proxmox server. That server is being upgraded and is currently unavailable. The only changes I made to the SNOM and other phones was to change the sip server address.

This sounds like it could be a NAT problem. Is extension 514 sitting behind a NAT router?

If extension 514 is indeed behind a NAT router, add some port forwarding rules on the NAT router to forward UDP ports 5060 and 10000:20000 to the phone. Setting the phone up to send a qualify at least once every 120 seconds (usually the default timeout for UDP connections in a NAT table) might also fix the problem.

Sometimes you have to go to the advanced section of a Snom phone and set the network port to 5060. It uses something weird by default. See if that makes a difference.
