Incoming Audio Lost

I have a PIAF box that was previously a trixbox. It has a TDM-400 card that I'm using for one POTS line. I have a Linksys PAP2 with two analog phone connected for the extensions.

I have an issue right now where answering a POTS call seems to end in one of three different states
1. The call completes fine. - 50%
2. There is no audio heard on the SIP extension. I can confirm that the dahdi trunk hears the incoming audio from the caller with dahdi_monitor however I can't hear it on the SIP extension. There is also no audio from the handset mic to the earpiece. - 25%
3. The call is answer, with no audio, however the other end still rings. If I hang up I might get another ring and then successfully complete the call. - 25%

Do calls between the two extensions on the PAP2 work?

Did you just convert it from the Green Machine or has it been running PIAF for a while and this just started happening?

If you have a SIP phone (or a soft phone), try adding it and see if it works reliably.

Did you make sure that you set the Sip Settings correctly, showing you outside IP and the Localnet settings?
