BUG IVR and audio stream fail to play

New install of latest GREEN with incredible PBX install.
it happens more often then not no audio is played. I see this in the log depending on what is happening and other times it works as expected.
I have restarted and it will happen not to play audio on the first call. then later it works OK.

Any ideas?

Decided to install g729 and have yet to see the issue come back. I am not too happy with the quality but it is better then not working at all.
If anyone finds a fix I would like to know...
