With the wide variety of providers and the limited markets they support, it may be better to state who provides service in your area and what experiences others have had with those providers.

But to answer your question as is below. I'm located in Melbourne, FL:
With my PBX on the Brighthouse Wideband Service I would notice that my call would get choppy for a second or two every few minutes.

I added AT&T DSL to the mix (Dedicating this line to the PBX and as a failover for regular internet) and the issue went away and I haven't had any issues since.

I am currently considering moving my PBX to my data center in Jacksonville...but this is still an ongoing internal debate =P

ATT DSL has been pretty good for my smaller customers. For my larger customers (and myself) I use Cbeyond

Do you mean a VoIP provider?
I've been using Voip.ms for years. Excellent quality, good support, and they have a bunch of POPs.

Regardless of the ISP, it is worth creating some simple QoS rules at the gateway. The importance of this step is inversely related to the available bandwidth.

QoS will prioritize time-sensitive data (voice, video) over the other network traffic, and will go a long way in curing audio issues when bandwidth is lacking.
