NO JOY No audio till after on hold

I've got a small system running in the cloud with 4 endpoints at a client location.
They can receive calls fine. When making calls, there is no initial audio when the call is answered until the calls is put on hold then the audio begins to work.

Its behind a sonicwall firewall, so I'm sure thats it, but not sure how to fix.
 
I appreciate the assumption that I didn't do these two things. I did though. I just can't seem to figure out where the issue is. I think it has to be in the sonicwall.
