NEED MORE INFO Phone extensions changing on their own!

I have a system running in Device/User mode (I know, not officially supported) which has been operating fine for about 6 months. Then the other day, my customer told me that the wrong phones were ringing when extensions were dialed. I went to Device settings, selected Ad-Hoc mode, and changed all the extension numbers to an arbitrary extension, then set them all back to what they should be, and selected "Fixed". Now they ring correctly, but I am puzzled as to why the assigned extension numbers would change on their own. Has anyone else had this experience? Know of a fix? Thanks.
 
Is there a remnant config file sitting on a server somewhere being picked up by a tftpboot process? Or are they Yealink and maybe incorrectly programmed in their RPS system (or equivalent for other vendors?)
 
This was a clean installation of the server for this customer, and I am using 5 brand-new Yealink T48 phones, which were never configured to another server. There should not be any of the conditions present.
 
Yealink has an manufacturers system they call RPS for configuration of out of the box new phones. If your phones were mistakenly put in there that might cause it. However, you likely would have noticed this immediately on boot up, so I'm more inclined to blame a firmware bug at this point. There are some of those that appear for Yealink from time to time (not terrible, but not zero errors either).
