PiaF 1.1 & TDM400 = Incoming, but no outgoing?

I've got a PiaF 1.1 install that I've slipped a TDM400 into the configuration... all went well & installed smoothly, nothing out of the ordinary, however:

I *can* accept inbound calls, but can *not* place outbound calls via analog lines.

When I try to dial out, I get a bunch of CHANUNAVAIL in CLI and am greeted with 'all-circuits-busy' over the phone. I've setup the trunks in FreePBX both as g0 and as 1,2,3 but neither made any difference. I've checked zaptel.conf and zapata.conf, and both look normal (again, nothing out of ordinary).

Now, all is well when they have registration w/ SIP Providers, but when that registration fails (like earlier today)... they have no analog failover for outbound calling.

Any ideas?!?

PiaF 1.1
Linux 2.6.18-8.1.15.el5
Asterisk 1.4.18.1
Zaptel 1.4.9.2

::update::

Adding 'w' to 'Trunks > Outbound Dial Prefix' didn't help either...

Code:
    -- Executing [s@macro-dialout-trunk:20] Dial("SIP/99-b7d6d1e0", "ZAP/3/18007425877|300|") in new stack
    -- Called 3/18007425877
    -- Hungup 'Zap/3-1'
== Everyone is busy/congested at this time (1:0/0/1)
    -- Executing [s@macro-dialout-trunk:21] Goto("SIP/99-b7d6d1e0", "s-CHANUNAVAIL|1") in new stack

IIRC zaptel 1.4.9.2 was notoriously bad for giving spurious battery alarms and other problems and thus not being able to dial out

it even segfaulted when presented with the automatic line testing signals that British Telecom send down some phone lines in this country!

check in /var/log/asterisk/full for any odd things, or just upgrade the zaptel version and see what happens..
