QUESTION PIAF 12 upgrade tampered file

Just upgraded yesterday. Today I get: Module: FreePBX Framework, File: /var/www/html/index.php altered

Is this something to be concerned about? I tried turning signature checking off, but still have the error.

Tips/tricks/suggestions?
