Just received this message from Vultr tech support:

Hello,

Unfortunately, Incredible PBX is no longer being offered, which would cause this issue to occur. Our system will not support unsupported marketplace apps.

If you wish to use this OS, you will need to deploy a new instance using a custom ISO. We apologize for the inconvenience.
