Eons ago I worked for a telecom company where we had Audiocodes gateways (TDM). Such great hardware, great platform. We used something called SS Monitor. I don't know if it was Audiocodes software exactly or 3rd party. In SSMonitor you had a view into how many calls total were on each gateway. With a break out (by colour) how many were in call setup (SS7 IAM) and how many were in actual conversation. You could click on the gateway additionally and get a live view of calls details as they set up and were up. This was real time, if anything maybe 5-10 seconds lag.

Is there something like that in SIP? I suppose one would be doing port mirroring to capture raw (tcpdump), or SNMP polling (where implemented) to get trunk statistics..

Any ideas?

You could monitor over AMI on port 5038 or perhaps implement asterisk SNMP where the mibs expose such data.

There is a trunkcheck.sh script to notify trunk failures see thread here
Not quite what the OP was asking for though, AMI exposes in realtime as connections are made but you will have to tally them yourself, SNMP can report the current count of open connections.
