Strange GoogleVoice issue

I have been using PBX-IAF for quite a while and really enjoy all the hard work you all have put into it. Really enjoying the free calling to US & Canada this year.
I would consider myself a linux n00b, so the easiest way to get my status screen on here was to type it in:
PBX in a Flash Version = 1.7.5.7 Running on *VIRTUALBOX*
FreePBX Version = 2.8.1.4
Running Asterisk Version = 1.8.6.0
Asterisk Source Version = 1.8.6.0
Dahdi Source Version = 2.5.0.1+2.5.0.1
Libpri Source Version = 1.4.12
IP Address = 192.168.11.176 on eth0
Operating System = CentOS release 5.7 (Final)
Kernel Version = 2.6.18-274.3.1.e15 - 32 Bit

The issue is with just one phone number. Everything else works fine.
So yesterday I noticed there was a number that I could no longer call. It rings once or twice, then goes to a busy signal, even though the number is not busy. My friends at the other end of the line can call me from that number, but I am unable to successfully answer the call. I can answer the call, it acts as if it is connected, we can't hear each other and they go to voicemail after a few seconds.
Then I tried calling from Gmail chat phone, and it did not work either. So I submitted a help request to GV, and they did fix it- partially. Now I am able to call via Gmail chat phone, but not through my PBX-IAF. What confuses me is that this is the only number that this happens with.

Any ideas?

All suggestions tips and tricks are truly appreciated.

BONUS: How do I change my config so that I can call ten digit numbers with a 1 in front? Right now I can only call if in the following format: 555-666-1234. I know there is a way to route calls with the 1 in front properly, I just don't know how.
 
My experience with google voice is hit and miss. We are so used to picking up the phone on a regular PSTN line, dialing and having no issues because the routing was so transparent. In the voip - wireline world it much more complex. There are some numbers when called on GV that just ring and ring even though they are not in service. GV also refuses to connect on some routes based on what they consider to be excessive charges for those routes. GV is not a 100% solution. I use a combination of PSTN lines, GV and other VOIP carriers to get to all the things I need to get to. I'm reaching the point where I just may eliminate GV altogether. If you are running a small business and making a lot of calls GV can save you a lot of money but for a 1 or 2 person household it starts to be more trouble than its worth.

Additionally most people do not consider it to be reliable enough to use as your sole, primary incoming line.
 
I do understand that it should not be used in a production environment as your sole, primary incoming line.
What I don't understand is why a call would work using the phone built in to Gmail but not through PBXIAF. And why it is only one number that displays such behavior.
As I understand it, PBXIAF uses Google Chat to send voice traffic through Google Voice. So why would only one specific number be prevented from interfacing properly between Google Chat and PBXIAF?

Edit: Just to clarify, I am calling the exact same number from Gmail's Google Chat as I am from PBXIAF. It works from Google Chat, it does not work through PBXIAF. Other numbers work fine and dandy through PBXIAF.
