I’ve recently been working with a client that has an XBlue QB system with IP9g phones. They’ve had complaints that caller ID rarely works anymore along with other features, but their biggest complaint is an issue with call forwarding. It will forward the call only about 25% of the time (no issues with any other outbound or inbound calling). I didn’t install the system but Xblue says it’s configured correctly for call forwarding. I noticed that there are 3 POTS lines into the server but there are 4 FXO configured in the server and they are all in use for outbound routes.

Curious if anyone thinks that 4th line is what’s creating the issues? If it is rolling over to that line to try and forward the call , it would disconnect since there’s no POtS lines there?

Not sure why you think that you should expect support for an "XBlue QB" system in a forum that is specifically for FreePBX based systems, is XBlue based on FreePBX ? from their website,

" if you need a hand setting up your system, you can always take advantage of XBLUE’s US-Based Technical Support.
For Assistance Call 866-925-8312 or Send Us a Message"

Xblue says everything is perfect, it’s the phone company’s fault. Phone company says everything is perfect, it’s Xblue’s problem. I’m currently trying to fix perfection!

Well 25% is 1 in 4 that's working and you have 3 FXO connected and one not. That's the other way around.
Disable the one that has no POTS connected and see what happens.
